Position#Summary The Transition of Care pharmacist for Home Health is responsible for working primarily with the high-risk home health population.#They will be responsible for working on a multi-disciplinary team within the home health department.# The clinical pharmacist is expected to provide comprehensive care at the following intervals of care, Start of Care, Resumption of Care, Discharge and following any physician appointments where potential clinically significant medication issues were identified by the care team.# The patient population will consist of home health patients with polypharmacy and or high-risk for rehospitalization.# This position is also expected to work in collaboration with the clinical pharmacy team as appropriate to ensure safe transitions of care. #Primary Position Responsibilities Completes comprehensive medication reconciliation for identified home health patients at start of care, resumption of care, discharge, and following physician encounters with significant medication changes. Provide medication education to high-risk patients. Communicates with multi-disciplinary team on compliance with drug regimen reviews. Provides education to home health staff on improving drug regimen review process and high risk medications. Participates in other duties as assigned. #Work Contact Group (Internal/External) HMC employees, providers, students, patients and families, visitors, vendors. #Reporting Relationships Reports to (position): Director of Pharmaceutical Services, with a indirect report to Director of Home Health Services Supervises (position(s): None #Qualifications Minimum Education: Required: Doctorate degree#of Pharmacy. Preferred: None Minimum Years of Experience (Amount, Type and Variation): Required: Completion of PGY1 residency or 3 years hospital with significant clinical experience. Preferred: PGY2 residency or 5 years hospital with significant clinical experience. License, Registry or Certification: Required: Current New Jersey Board of Pharmacist License. Preferred: Specialty pharmacy certification (e.g. BCPS, CGP). Knowledge, Skills and/or Abilities: Required: Pharmacy skills serving patients of diverse age and disease states; Competence in required computer programs (clinical system, ADC, parenteral documentation,#etc), MS Office Proficient;#Able to interact with patients and comfortable being in different environments such as the patient#s home.
