Overview:APP-EG1Capital Health is Hiring Nurse Practitoners and Physician Assistants!Varied shifts available * Varied Statuses available *Positions available in Trenton and PenningtonResponsibilities:Performs physical assessment of patients to identify priority care needs. Documents patient assessment and teaching and learning needs. Obtains detailed nursing history and identifies patients at physiologic and psychological risk. Consults with interdisciplinary team regarding patient assessment findings.Assists with admission work-ups and/or consults to facilitate care across the continuum.Initiates, monitors, alters and order treatments including but not limited to device placement, physical and occupational therapy, home health referral, and discharge planning.Re-evaluates the patient throughout the hospitalization and modifies plan of care as needed in collaboration with clinical staff. Completes transfer of care notes when transferring patients to another medical service.Performs diagnostic and therapeutic procedures as indicated for diagnosis and management of problems.Completes wound assessments, initiates basic skin care orders. Orders laboratory, radiological and diagnostic studies that will assist with the medical management; explain the necessity, preparation, nature and anticipates effects of treatments, and diagnostics and therapeutic procedures to the patient.Contributes to development and revision of patient care standards and polices as needed to optimize clinical practice.Performs procedures necessary to implement plan of care based on care standards and protocols.Develops plan of care across the continuum and exercises discretion and judgement when revising based on physiologic and psychosocial findings. Orders tests and consults as required by plan of care and reviews test results. Reviews patient plan of care with interdisciplinary team. Discusses plan with patient and family.Demonstrates the ability to review and evaluate the plan of care for effectiveness of desired outcomes.Documents all patient care activities as per documentation standards. Evaluates outcomes of documentation and contributes to performance improvement initiatives through medical record review.Requirements:Physician Assistant -Graduate of an accredited physician assistant program, approved by the National Commission of Certification of Physician Assistants (NCCPA)One year experience working with primary care management or acute care preferred.NJ Physician Assistant License.orNurse PractitionerBachelor's degree in nursing and Master's degree in advanced clinical practice or graduate of post Master's certificate program for nurse practitioner.Three years clinical nursing experience. Two years experience as Nurse Practitioner preferred.Advanced Practice Nurse - NJ LicenseFrequent physical demands include: Sitting , StandingOccasional physical demands include: Walking , Climbing (e.g., stairs or ladders) , Carry objects , Push/Pull , Twisting , Bending , Reaching forward , Squat/kneel/crawl , Wrist position deviation , Pinching/fine motor activities , Keyboard use/repetitive motion , Talk or HearContinuous physical demands include: Taste or SmellLifting Floor to Waist 20 lbs. Lifting Waist Level and Above 0 lbs.
