Job Description:
Job Summary:
The patient navigator collaborates with the primary care provider,
all members of the health care team, and community organizations to
accept referrals and manage cancer screening/oncology caseload.
She/he works with other healthcare professionals to establish and
maintain a climate of mutual respect, dignity, diversity, ethical
integrity, and trust.
As part of the cancer outreach program, the Oncology Lay Navigator
supports and executes the interventions and strategies in a
particular territory to impact coordination of cancer care,
integration with primary care and other specialty care,
patient-centered communication and engagement in care and
psychosocial and other supportive care with the goal to improve
access to high-quality cancer care for underserved populations.
The Oncology Lay Navigator supports care that is culturally
sensitive and linguistically appropriate, building collaborations
with trusted community-based partner organizations that can help
address the social determinants of health and the innovative use of
digital tools to reduce barriers to care.
The Oncology Lay Navigator position is a grant funded position with
Alliance for Equity in Cancer
Job Requirements:
Associates degree in Health Care, Human Services or a related field
or equivalent work experience.
The candidate must possess a valid driver's license and be willing
and able to drive a car.
Minimum of 2 years of health care in an office/clinic/hospital
setting or human services experience preferred
Community Health Worker based care with a focus on culturally
sensitive and linguistically appropriate assessment experience
preferred
Previous experience in presenting health education materials to
individuals and small groups.
Ability to effectively communicate preventive health and health
management philosophies through community outreach
Spanish speaking preferred
Must have strong interpersonal skills and the ability to work with
a vast array of internal and external health care providers.
Demonstrates ability to be confident, personable, self-motivated.
Demonstrates excellent communication skills, knowledge, judgment
and leadership in a multidisciplinary environment. Knowledge of the
relevant of health information and appropriate documentation
methods. All patient related hospital departments, patients,
families, visitors, nursing personnel and medical staff.

Job Functions
Business Acumen / Operational Effectiveness
Build and maintain relationships with key staff to build patient
referral mechanisms and promote patient navigation services and
high-quality cancer care.
Complete comprehensive assessments of the barriers to quality
cancer care that contribute to disparities and identify
information, resource and emotional support needs of newly
diagnosed and medically underserved patients, survivors, and
caregivers. Engage patients and families in creating potential
solutions to financial, practical and social challenges.
Effectively link patients referred from the community to resources
that can improve care coordination and timeliness to treatment with
Oncology Nurse Navigator ONN
Communication
Help patients communicate the complexities of the healthcare system
by assisting with practical problem solving related to concrete
needs such as lodging and transportation, and other services that
decrease barriers to treatment. Identify appropriate and credible
resources responsive to patient needs, practical, social, physical,
emotional, spiritual, taking into consideration reading level,
health literacy, culture, language and amount of information
desired. Effective in getting ideas accepted through written and
oral communication.
Leadership
Support annual departmental goals and objectives based on
philosophy and overall institutional goals.
Maintains department specific policy and procedures, Quality
Assurance Plan, Safety Program, as well as assuring compliance to
hospital-wide policies and guidelines of regulatory agencies.
People Management
Establish effective procedures for monitoring processes and
evaluates. Demonstrates an ability to perform accurately and
precisely in decision making during routine and crisis situations.
Strives to instill an awareness of sensitive patient relations in
interactions.

Operates with a high degree of autonomy and authority while working
within the framework of RWJBH s philosophy and policies.
