Job Description:

Vitality NP Services is seeking a Nurse Practitioner to become a part of our team!Full time/ Part time position open for NP in a -skilled nursing facility in Bridgewater, NJ.Offering flexible hours and days. No weekend, evenings or on call required!Responsibilities:

Assessment of all recent admissions and acute issues of both subacute and long-term care patients
Prevention of needless hospital transfers through early assessment and interventions
Communication and collaboration with primary care and specialistsQualifications:
- Previous RN experience - -NEW NP GRADS WELCOMED!
- Ability to work as an interdisciplinary team member
- Good critical thinking/ assessment skills and the ability to function in a fast paced environment
- Excellent written/verbal communication skills and computersPowered by JazzHR
