Job Description:
Our confidential client is looking for a full-time Therapist to
work onsite in Mercer County, NJ. - RESPONSBILITIES:
Plans, organizes and implements
clinical counseling services to consumers and families. - Plans for
discharge and aftercare. - Maintains contact with referring
agencies, treatment team providers, and related people, and with
the community resources used for referral. - Maintains treatment
records. - Assesses and interprets data about the consumer's status
in order to identify each consumer's needs and provide the
appropriate care, including age-related care, needed by the
consumer group.
