Job Description:

The Palliative Medicine Nurse Practitioner provides consultation in palliative medicine to meet the needs of a patient as requested by their referring physician. The nurse practitioner is a specialist in assessing the patient's condition and developing a plan of care to improve patient understanding of their complex illness, adherence to course of treatment, and quality of life in context of patients values. The nurse practitioner also develops a plan of care to manage complex disease-related symptoms including emotion, physical, and existential components. Importantly, the nurse practitioner communicates with all members of the patients interdisciplinary care team to insure proper coordination of the best care possible.See what it's like to work for KeystoneCare: https://www.youtube.com/watch?v=DyEH9buz_VcKeystoneCare is an Equal Opportunity EmployerPrimary Job Responsibilities:Community-based position: providing palliative medicine to patients wherever they call homePerform palliative medicine consults utilizing strong assessment and evaluation of the patient in context of their social support systemCollaborate with the patient's physician, other health care team members, the patient and their family in developing realistic care expectations based on goals of careFacilitate patient and family decision-making regarding treatment decisionsRecommend pharmacological and non-pharmacological interventionsMonitor patient's response to treatment modalities and revise plan as neededCommunicate effectively with patients and families for understanding of diagnosis, prognosis and treatment plansDevelop and maintain effective working relationship with providers, staff, community leaders and organizations to promote positive image; heighten awareness and promote collaborationConduct consults and follow up and complete clear and accurate documentation as required in a timely mannerConduct hospice face-to-face encounters as needed and complete documentation in a timely mannerRequirementsGraduate of an accredited school of nursing and nurse practitioner programCurrent license in good standing to practice in the Commonwealth of PennsylvaniaMinimum of two years RN or NP experienceACHPN certification required or be able to obtain certification within 6 months of hireMaintains clinical and professional competencePossess a controlled substance license in the Commonwealth of PennsylvaniaPossess the ability to communicate effectively with all members of the care teamPossess good organizational skillsCurrent driver's licenseReliable transportation and automobile insurance requiredProof of COVID-19 Vaccination or an approved Medical or Religious exemption.Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption.BenefitsHealth Care Plan (Medical, Dental & Vision)Flexible SchedulingRetirement Plan (403B)Life Insurance (Basic, Voluntary & AD&D)Paid Time Off (Vacation, Sick & Public Holidays)Access to Short Term & Long Term DisabilityTraining & DevelopmentTravel reimbursement between work sitesby Jobble
