Job Description:
Carnegie Post Acute Skilled Nursing Facility is seeking a
dedicated and compassionate Licensed Practical Nurse (LPN) to join

As an LPN at Carnegie, you will be integral in our mission to
deliver top-tier, patient-centric healthcare and support in
rehabilitation and skilled nursing services. Provide basic bedside
care to patients including administering medications, checking
vital signs, and giving injections Assist in the assessment,
planning, implementation, and evaluation of patient care
Communicate with patients and their families, offering emotional
and support resources Ensure the health, comfort, and safety of
patients Monitor patients' health changes and report to registered
nurses and doctors Maintain accurate patient records and adhere to
HIPAA regulations Participate in development and review of care
plans Proficiency in administering medications and monitoring
vitals Strong communication and interpersonal skills Capable of
handling physical demands of patient care Excellent organizational
and time-management skills Medical Insurance Dental Insurance
Vision Insurance Current LPN license in the state of practice CPR
certification
