Job Description Job Description /n Plans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease. Provides nutritional assessment, education and counseling to individuals and/or groups. Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility. /n Job Responsibility /n /n /n - Plans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease. /n - Assesses nutritional needs and considers patients' cultural, educational, and learning issues. /n - Provides individualized nutrition education to patients, their families, and/or significant others based on their needs assessment. /n - Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility. /n - Communicates patient information to referring providers and appropriate staff in a timely manner. /n - Documents nutrition information discussed, written materials provided, and evaluation of patient’s receptiveness and comprehension of the education provided. /n - Performs related duties as required. Duties not mentioned here, but considered related are not essential functions. /n /n Job Qualification /n Master's Degree in Food, Nutrition or Dietetics, required. Registered Dietitian, required OR Bachelor's Degree in Nursing, required. Current license to practice as a Registered Professional Nurse in New York State, required. /n Current Diabetes Educator Certification, required. /n Minimum of three (3) years progressively responsible, related experience to ensure familiarity with scope of program. /n Experience with Endocrinology, preferred. /n Ambulatory experience, preferred. /n *Additional Salary Detail /n The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.
