Qualifying candidates are eligible for up to $7,000 sign on bonus.
Candidates holding a NJ Choice Certification are eligible for an up
to $10,000 sign on bonus.Prefers candidates residing in either
Mercer or Middlesex County NJStandard working hours Monday -Friday
8am -5pm EST.RN Assessor is responsible for telephonic and/or face
to face assessing with members to evaluate the medical needs of the
member to facilitate the member's overall wellness. Addressing
requests for services such as adult medical daycare, pediatric
medical daycare, personal care assistant, nursing facility
custodial requests, Personal preference program and MLTSS.. 10-15
assessments minimum per week. Assessments take into account
information from various sources to address all conditions
including co-morbid and multiple diagnoses that impact
functionality. Assessments include the member's level of work
capacity and related restrictions/limitations. Using a holistic
approach assess the need for a referral to clinical resources for
assistance in determining functionality.Consults with supervisor
and others on team in overcoming barriers in meeting goals and
objectives. Utilizes case management processes in compliance with
regulatory and company policies and procedures.Utilizes
interviewing skills to ensure maximum member engagement and discern
their health status and health needs based on key questions and
conversation. The RN Assessor will also be expected to mentor new
hires, once, a level of proficiency has been attained in their
role. Successful completion of the NJ Choice certification is a
contingency of employment. As per NJ Division of Aging Services,
Office of Community Choice Options guidance, an exam score of 80%
or higher is required, along with successful completion of the
State training modules, and field mentoring component."
Required Qualifications
-Minimum 2 years of clinical practice experience clinical practice
experience, e.g., hospital setting, alternative care setting such
as home health or ambulatory care required.-Active and Unrestricted
RN license in NJ required-Willing and able to travel up to 75 -100%
using your own vehicle to meet members face to face in Mercer and
Middlesex Counties of NJ and surrounding areas/counties.-Valid
Driver's License Required. - Must be comfortable completing Field
based assessments of people of all different and socioeconomic
status.- Successful completion of the NJ Choice certification is a
contingency of employment. As per NJ Division of Aging Services,
Office of Community Choice Options guidance, an exam score of 80%
or higher is required, along with successful completion of the
State training modules, and field mentoring component."

Preferred Qualifications
Case management and/or discharge planning experience
preferred-Managed Care experience preferred-Crisis intervention
skills preferred-NJ Choice certification-Great clinical
Interviewing skills.-Comfortable in any socio -economic
settings.
Education
Associated of Nursing Required. Bachelor's Degree Preferred.
