Must have behavioral-mental health experience

Must be charismatic enough to engage with patients and educate them about the importance of enrolling in preventive care through a telehealth platform

Job Summary:

looking for a part-time LCSW to provide direct mental health care services with physician collaborationto patients remotely(via phone, FaceTime, etc.). The selected candidate will provide services in compliance with and accountable to the standards, rules and policies and procedures of the practice. All services must comply with Medicare regulations and standards.

Essential duties and responsibilities:

Provides outreach via phone calls, FaceTime, etc. to engage senior citizen clients and their families into our program
Reviews cases with the social work supervisor, PCP and Nurse Practitioner to discuss findings and to devise plans of patient care covering medical, nursing & behavioral health aspects.
Participates with NP and PCP in the delineation of standard treatment regimens in the management of individual health programs.
Participates in weekly case conferences with PCP team consisting of NPs, social workers and psychiatric NPs-psychiatrists
Reports to medical supervisor and actively seeks out supervision for challenging aspects of care and-or crisis management in a timely manner.
