*Overview*This is a short-term contract that ends on 3/31. Ideally ASAP start.We are seeking a dedicated and skilled Physical Therapist (PT) to join our team. The ideal candidate will have a strong background in physical therapy and a passion for helping patients achieve their rehabilitation goals. This role involves working with a diverse patient population, including geriatrics and outpatient clients, to develop and implement effective care plans tailored to individual needs.*Responsibilities** Conduct comprehensive assessments of patients' physical abilities and limitations.* Develop personalized care plans that incorporate evidence-based practices and patient goals.* Implement various therapeutic techniques, including trigger point therapy and discharge planning, to facilitate recovery.* Collaborate with other healthcare professionals to ensure a holistic approach to patient care.* Educate patients and their families on treatment plans, exercises, and home care strategies.* Monitor patient progress and adjust treatment plans as necessary based on ongoing evaluations.* Maintain accurate documentation of patient treatments, progress notes, and discharge summaries.*Experience** Proven experience in physical therapy, particularly in home health or community care settings is preferred.* Strong knowledge of physiology and the human body systems relevant to rehabilitation practices.* Familiarity with geriatric care principles and outpatient therapy techniques is advantageous.* Ability to create effective care plans that address both physical and emotional aspects of recovery.* Excellent communication skills to interact effectively with patients, families, and interdisciplinary teams.Join our team and make a meaningful impact in the lives of our patients through your expertise in physical therapy!Job Types: Full-time, Contract, TemporaryPay: $55.00 - $60.00 per hourExpected hours: No more than 40 per weekBenefits:* 401(k)* 401(k) matching* Dental insurance* Flexible schedule* Health insurance* Vision insuranceMedical Specialty:* Geriatrics* Physical & Rehabilitation MedicineSchedule:* 8 hour shift* Monday to FridayWork Location: In personby Jobble
